i use COMMAND OPTION+CTRL+SHIFTS
this seems to work well.
Also, i have noticed that if you are selecting text from a web page in Safari and you highlight any graphics along with the text you want to have 'read' to you, it will sometimes not function. I think it sees the graphic and thus ignores selection. But i am not sure really. I just try to avoid making a selection that has anything but text in it.
